End of life care

Information

If your parent or loved one has been told that they may not get better, end-of-life care and support services, also known as palliative care, provide treatment and assist with managing pain and other symptoms resulting from a terminal and/or complex illness while they are approaching the end of their life.

With a complex and terminal illness, symptoms may need to be controlled and managed where possible. Our care team can offer a clinical nurse-led end-of-life service at home to offer the full spectrum of medical, practical, personal care and emotional support required during this time.

We are usually asked to become involved at an early stage to help with medication and practical help, sometimes also helping with getting out and about while health still permits this.
Because we bring health and social care professionals to your family member or loved one's home, there is no need to be worried about whether they will be able to pass away at home, which is often what people would prefer.
